A copy of the details submitted will be sent in PDF format as an email attachment to Compendium and to yourself. Simply save this attachment to your PC or print it out for your records To print a form in PDF format, click on this icon alongside the title of the relevant form. Fill out the form, sign, and fax to your Compendium office.
